If you think your patient's medical condition may benefit from Spravato® (Esketamine) or ketamine treatments and want to refer to us, we are happy to help!

Referring is easy. Complete the questionnaire then click "Submit". You may fax all recent and relevant medical records to (916) 244-3836 or email it to us at [email protected]. That's it!

After that, we will contact your patient to schedule a consultation and keep you informed on their progress and any significant developments. We will always ask them to continue your current treatment plan. Thank you for your collaboration in providing comprehensive care.
